health records (PHR) give emergency personnel needed information to save
your life. Your personal health records should contain a list of
your medications, allergies, medical conditions such as diabetes or
heart problems, emergency contact information for your friends and
family, and instructions for medical personnel such as DNR (Do Not
Resuscitate) orders.
